Providers - Update Your Information
In order to keep our records up-to-date, we request that our participating
providers notify us anytime an information change occurs.
If you are a Devon Health Services, Inc. preferred provider and wish to update
your information you may select one of the links below and either submit your
change online or print out our
form 
and
fax your updated information.
If you have changed your Tax ID Number, please contact us at 866.498.4773 or
nleh@devonhealth.com to obtain a Tax ID Addendum form. We cannot
accept Tax ID changes online.
If you are updating your credentials, please fax them directly to our
Credentialing Department at 610.768.4539.
