How to Nominate a Provider

Occasionally, our members find that a provider that they would like to visit is not yet participating in the Devon Health PPO Network. Should this be the case, we are prepared to take action. Our Network Development Team is a group of highly trained representatives whose focus is to build the network to meet the needs of our members.

There are several ways to nominate your provider.

  • Call us at 800.431.CARE (2273)
  • Access our online nomination form
  • Fax your request directly to the Network Development Department at (800) 221-0002
  • Email your nomination to wbruce@devonhealth.com
  • Please include the following information in your fax/email, or have it handy when phoning in your request:
    • Your name and contact information
    • Your employer group and/or insurance company name
    • Provider name
    • Provider practice name
    • Provider address
    • Provider phone number
    • Provider specialty

Work on your nomination will begin immediately.

  1. A Network Development Representative will be assigned your request.
  2. The Representative will contact your provider’s office and speak directly with your provider or the office manager. Your needs will be explained to the office and your name will be mentioned.
  3. An application and contract will then either be mailed or faxed to your provider.
  4. Follow up calls will be made as necessary until the provider either mails in the completed application or refuses participation.

NOTE: No matter how beneficial our offer is to the provider, some offices simply will not join the network. In this instance, a member of our staff will be more than happy to help the requested patient find another provider who does participate in our network.

How can you help?

Contact the provider’s office to explain that you have nominated your doctor for participation in the Devon Health PPO Network. It is very helpful to our Network Development Representatives when their call is expected. In addition, as a patient, your business is very important to your provider. Explain your needs to your doctor, as this will help him understand why he has been nominated.

What is the best way to follow up on a nomination?

Your provider nominations are very important to us and they are handled with the highest priority. The easiest way for you to receive information about the progress of your nominated provider is to check with the provider himself or herself.

NOTE: The more often a provider is reminded of both a patient’s affiliation with Devon Health and the fact he is not participating, the more likely it will be that the provider will seriously consider our network.

If we have your name, phone number, and insurance carrier, we will periodically call you to inform you of our progress. However, if you have not heard from us, we suggest that you place a call to our Customer Service Department at least 30 days after you submit your nomination. One of our representatives will be able to give you details on the progress of your nomination.

Ultimately, patience is key. Our goal is to recruit all requested providers into our network, but it can take time. A provider’s participation can occur quickly, very slowly, or unfortunately, not happen at all. Devon Health can only work as swiftly as the provider’s office will allow. However, we guarantee that we will make every possible effort to fulfill your request.
